HerzMobil Tirol brings many benefits for patients and the healthcare system

The HerzMobil Tirol care program is coordinated and implemented by the Tyrolean State Institute for Integrated Care (LIV). Every year, around 350 patients are cared for by HerzMobil Tirol after being discharged from hospital. HerzMobil Tirol is financed by the state of Tyrol, the social insurance institutions and funds from the Tyrolean Health Fund.

HerzMobil Tirol currently employs 65 network doctors and 17 nursing staff. The care provided by HerzMobil Tirol not only involves close monitoring of health values, but also supports patients in changing to a healthier lifestyle.

The successful HerzMobil Tirol concept is constantly being developed further. A new traffic light system for the participating network doctors is intended to further improve the quality of therapy and, in particular, the adjustment of medication. “The traffic light system shows the treating doctors the quality of the current therapy on a daily basis. This allows for an even more targeted response and optimization. We are currently launching a new study in Tyrol and Styria to scientifically test whether we can work even more efficiently with this new system,” explains Gerhard Pölzl, Medical Director of HerzMobil Tirol.

ADAPT project: further improving the quality of care through telemedicine

The HerzMobil program has been implemented in Styria since 2017. Work is now underway on a software module to further improve care for heart failure patients. The ADAPT project is funded by the Styrian Health Fund as part of the Digital!Healthcare funding call, implemented by telbiomed Medizintechnik and IT Service and enables even more targeted medication dosing. ADAPT stands for Automated Dose Calculation and Display for Heart Failure Patients in a Telemedical Care Program.

ADAPT is to be integrated into HerzMobil, enabling the care team to compare the current medication dosage with the guideline recommendations at a glance. The guideline-compliant dosage should further improve patient care.

Did you know that ….

around 60 million people worldwide suffer from heart failure?
around 300,000 people in Austria alone suffer from heart failure?
up to 50 percent of patients die within 5 years of being hospitalized for heart failure?
there are around 24,000 hospitalizations per year due to the diagnosis?

These hospitalizations not only worsen the prognosis of patients suffering from heart failure, but also mean considerable costs for the healthcare system.

The telemedical care program HerzMobil was developed to tackle this problem and improve the quality of life of people with heart failure.

HerzMobil leads to better management of the disease and fewer hospital stays. Further advantages of the telemonitoring program:

Relief for the inpatient sector
Reduction in healthcare expenditure
Reduction in avoidable hospital admissions
Reduction in mortality (25.7% to 10.0%)

HerzMobil is currently financed by the respective regional health funds and social insurance in Tyrol, Styria, Carinthia and Lower Austria. The respective establishment of the care program is individually adapted to local conditions and implemented in coordination with each federal state.

Through the use of the program in standard care, the experience and proven benefits are increasing, as evidenced by numerous studies and evaluations, which is why HerzMobil is increasingly establishing itself as a potential telemedical care program for patients with heart failure and nationwide use in Austria.

Together with our development partner AIT Austrian Institute of Technology GmbH, we develop and configure telehealth software solutions for various indications and specific clinical pictures.

WINNER OF THE STARTUP PITCH CONTEST OF THE PREAVENIRE DIGITAL HEALTH SYMPOSIUM

We are delighted to have won the Startup Pitch Contest at the 6th Praevenire Digital Health Symposium, which took place from April 18 – 19, 2024 at the Hotel Le Méridien, Vienna.

The TBM CareManager can be used for many different indications. As a reference application, we mentioned HerzMobil in our pitch – a multidisciplinary, telemedical care program for patients with heart failure. HerzMobil Tirol as an example of best practice in the position paper of Janssen-Cilag GmbH

In mid-February 2024, the pharmaceutical company Janssen-Cilag GmbH published an open house position paper entitled “Value Based Healthcare: Aligning healthcare with patient outcomes”, which was developed together with interdisciplinary experts from the healthcare sector. In the position paper, the authors formulate demands for policymakers to further develop the German healthcare system in line with Value Based Healthcare (VBHC).

Value-based healthcare is based on the benefits and the health achieved by the patient. The quality of the outcome is compared with the costs in order to achieve the best possible patient benefit. The guiding principle is “outcome over cost”.

We are delighted that the authors have cited the HerzMobil Tirol program as a best practice example for the national and international arena! Congratulations to our long-standing partner, the LIV Landesinstitut für integrierte Versorgung Tirol and the entire HerzMobil team for their pioneering work in patient care.

LebensGroß is a new cooperation partner of telbiomed

The “DiLeLi” project by LebensGroß was chosen as the winner of the “Digital!Healthcare” funding call by the Styrian Health Fund.

In 2021, LebensGroß GmbH opened Austria’s first day center for people with eating disorders (LeLi) in Graz. The LeLi day center offers unique, interdisciplinary care in everyday life to avoid an inpatient stay or to return to everyday life after a period in a clinic.

In addition to the existing care services, “DiLeLi” is being developed and tested as an innovative communication solution for patients and the treatment team.

The telbiomed team is very much looking forward to working with the colleagues from LebensGroß and the AIT Austrian Institute of Technology and is confident that “DiLeLi” will support the trusting cooperation in the therapy network and improve the care of people with eating disorders.

We are delighted about the positive funding approval from the Styrian Health Fund for the ADAPT project

The Styrian Health Fund supports innovative “Digital! Healthcare – Project” and we are part of it! Out of 29 projects submitted, only six met the requirements of the expert jury. We are delighted that the “ADAPT” project we submitted was chosen as one of the winners. We signed the funding agreement on December 1, 2023. The aim of the R&D activity is to develop a decision support tool for doctors for guideline-compliant medication for heart failure together with our R&D partner AIT Austrian Institue of Technology GmbH.

Telbiomed at the Grazer Marathon

Together we ran the distance of 42.5 km as a team of 4 and made the 32nd place in the discipline “Hyundai Staffelmarathon” in the category mixed with a total time of 3 hours and 40 minutes. We are more than satisfied with our performance and are already looking forward to next year.

Opportunities and challenges of telemedicine-supported disease management programs

In the August/September 2023 issue of Springer‘s magazine “Das österreichische Gesundheitswesen ÖKZ”, you will find an exciting article by Günter Schreier and Angelika Rzepka on the opportunities and challenges of telemedicine-supported disease management programs.

The article describes the advantages of integrated care processes for patients, service providers and payers, how treatment pathways can be supported by telemedicine and the possibilities that the AIT Telehealth infrastructure creates for future areas of application. Of course, there is also up-to-date information on HerzMobil.

6th anniversary of telbiomed

Yesterday evening telbiomed celebrated its 6th anniversary in the Sudhaus in Graz.

Telbiomed, founded in 2017, was able to gain a lot of experience in the past 6 years, establish new cooperations, celebrate successes, expand our knowledge in the field of digital health and establish new care programmes and treatment paths for the care of chronic diseases together with our application partners.
We are grateful for our employees, business partners, customers and for the good strategic cooperation with the AIT Austrian Institute of Technology GmbH.

As part of a partnership with the eHealth Institute at FH Joanneum, eHealth graduates can gain practical experience at telbiomed in the field of telemedicine and digital health and complete their internship with us. There is also the opportunity to work on current and practical topics as a Bachelor’s or Master’s thesis.

Go2Market Australia

Telbiomed and AIT Austrian Institute of Technology intend to expand the partnership with the University of New South Wales (UNSW) and the Prince of Wales Hospital (PoWH) in Sydney.
So far the focus has been on research projects and clinical trials, but during the pandemic in the South Eastern Sydney Local Health District (SESLHD), more than 8,000 Cov19 cases were monitored using the KIT Telehealth Platform. Results of the partnership were presented on 27th of July in the Special Session “Digital Health Innovation – from Telehealth to Data-Driven Health and Care” at the IEEE Engineering in Medicine and Biology Society Conference (IEEE EMBC 2023) by Günter Schreier (Senior Scientist, AIT), Sze-Yuan Ooi (Cardiologist, PoWH), Peter Kastner (CEO, telbiomed) and Ahmadreza Argha (Scientist, UNSW) in Sydney.

In the following visit of the PoWH’s Virtual Care Centre, the need for Digital Health Innovations to address the challenges of health systems was discussed. Improving care for the chronically ill and supporting follow-up after inpatient treatment are also high priorities in Australia.

AIT and TBM as exhibitors at the 13th Austrian Health Economics Congress (ÖGWK)

On 22nd and 23rd of June 2023, the 13th Austrian Health Economy Congress (ÖGWK) took place in Vienna. Together with the AIT Austrian Institute of Technology, we attended the congress as exhibitors, where we welcomed Prof. in. Dr. in Elke Guenther (HoC for Health & Bioresources) as well as one of the new managing directors of the AIT, Mr. Alexander Sveijkovsky. Dr. Anton Dunzendorfer (Head of Competence Unit) presented in his session “Telemedicine – technical, organisational and economic benchmarks” the results of the cost-effectiveness analysis regarding HerzMobil Tirol and the components of the KEEP-IN-TOUCH (KIT) software.

Due to the exciting presentations, panel discussions and discussions the 13th ÖGWK was a complete success and we are looking forward to next year.

 

 

HerzMobil Styria as a new exhibitor at Haus Esther

The House Esther was opened in June 2022 in cooperation with the Geriatric Health Centres Graz (GGZ) and the Humantechnology Styria Cluster (HTS) as a centre for age appropriate assistance systems with a showroom for assistive technologies.

Since Monday 12th of June 2023, HerzMobil Steiermark with the KIT Telehealth solution by telbiomed/AIT is exhibited there for visitors and interested parties.

“HerzMobil Steiermark is a comprehensive care programme that saves patients with heart failure (cardiac insufficiency) travel and waiting times through the use of telemedicine – with the same level of care security.”

The handover of the information material and the demo set took place during a presentation by the HerzMobil coordination team Bernadette Taucher, Dr. Stefan Pötz (KAGes) and Peter Kastner (telbiomed), followed by a guided tour of the AAL showroom by Dr. Wolfgang Kratky.

New publication "Integrated Telehealth Infrastructure for Outpatient Stroke Pathway Tyrol".

StrokeNet is an IT solution for the coordination and documentation of integrated, home-based care for patients after an acute stroke event.

In the new publication “Integrated Telehealth Infrastructure for Ambulatory Stroke Pathway Tyrol“, written by authors from AIT, LIV Tirol, UMIT Tirol, Land Tirol and telbiomed, the telehealth infrastructure of StrokeNet is explained and first benchmarking results are presented. The work was presented at dHealth 2023 on 16th & 17th of May 2023 in Vienna.

Success Story of IHE Austria - Standardised Telemonitoring Episode Report for ELGA

Learn more about the standardised telemonitoring episode report for ELGA in the new success story from IHE Austria.

With the telemonitoring episode report, telemedical care programmes for chronic diseases are connected to the electronic health record (ELGA).
The episode report was developed by telbiomed Medizintechnik und IT Service GmbH and AIT Austrian Institute of Technology GmbH with the support of HL7 Austria, ELGA GmbH and other partners. The report is being used for the first time in the telehealth services HerzMobil Tyrol & HerzMobil Steiermark, which also commissioned the project.

 

 

Positive completion of MDR certification for software as a medical device at the AIT Austrian Institute of Technology - our strategic R&D partner

Congratulations to the Health Information Systems team at the AIT Center for Health and Bioresources for the successful MDR certification of KITMed.

The medical device KITMed (Class IIa) is used for decision support in telehealth services for patients with chronic diseases such as cardiac insufficiency, hypertension or diabetes mellitus. KITMed enables an automated comparison of the vital parameters transmitted by the patient (blood pressure, heart rate, body weight, blood sugar, etc.) in relation to the limit values specified by the doctor. The KITMed report indicates under- and overshoots of individual limit values and serves to support professional users in telemedicine centres.

With the successful certification, the AIT was able to complete the complex conversion from the previous Medical Device Directive MDD 93/42EWG to the new Medical Device Regulation MDR 2017/745 within the legally stipulated transition period (26 May 2024).

Cardiology Network Burgenland Meets: Heart Failure Network OST

On October 8, 2022, under the direction of Primar Prof. Dr. Rudolf Berger and Primaria Prof. Dr. Janette Strametz-Juranek, the event “Cardiology Network Burgendland Meets: Heart Failure Network OST” took place at the Kultur Kongresszentrum in Eisenstadt. In addition to many exciting presentations on current guidelines, treatment options and case discussions, Peter Kastner reported on initial experiences with the HerzMobil care program in Styria.

From 28th to 30th of September 2022 the Joint Annual Conference of the Austrian (ÖGBMT), German (VDE DGBMT) and Swiss (SSBE) Societies for Biomedical Engineering will take place in Innsbruck. The three-day program includes scientific lectures, focus sessions, panel discussions and scientific poster presentations from the broad range of topics in biomedical engineering.

KABEG LKH Villach wins INTEGRI Award for "Mobile Patient Care" initiative

Congratulations to the team of LKH Villach for the INTEGRI AWARD for the initiative “Mobile Patient Care”.

The LKH Villach has combined several objectives in one organizational form in order to optimize the integrated care. On the one hand the care by a mobile palliative team was mentioned as an optimization as well as the ambulatory geriatric mobilization in the residential environment of the patient. In addition, HerzMobil Kärnten was listed as a further improvement. HerzMobil Kärnten provides telemedical care for cardiological patients.

dHealth 2022

The team of telbiomed had many intensive discussions at dHealth 2022. At the information booth of telbiomed and  AIT, visitors could inform themselves about the latest developments. HerzMobil was presented by experts in different sessions and and was also highlighted and discussed from different perspectives as a best practice model for telemedicine-supported integrated care.

HerzMobil goes ELGA - Report at the DMEA in Berlin

Connection of telemedical care programs of chronic diseases to the electronic health record (ELGA)

telbiomed and the AIT Austrian Institute of Technology developed a standardized telemonitoring report with the support of HL7 Austria and ELGA GmbH as well as with other partners. For the first time, the report is used in the telehealth services HerzMobil Tyrol & Styria.

Internationally, this innovation is being presented by Univ.-Doz. DI Dr. Günter Schreier (AIT) in the context of the DMEA 2022 in Berlin …

REMARKABLE RESULTS OF HERZMOBIL TIROL

HerzMobil is based on the fact that health data (blood pressure, heart rate, body weight, well-being, medication) is recorded electronically on a daily basis. Patients are trained by nursing staff and cared for together with doctors in the care network. Medical professionals can use the data to optimize therapy (e.g. in terms of medication and dosage) in a timely manner.

A recent evaluation of HerzMobil in Tyrol showed a 61% lower annual mortality rate and a 34% lower readmission rate for heart failure patients compared to standard care.

The HerzMobil telemedical treatment pathway is already being used in 3 federal states (see www.herzmobil.at). The IT service for the telehealth service is provided by TELBIOMED in cooperation with AIT Austrian Institute of Technology.

RAPID ASSISTANCE FOR COVID-19 QUARANTINE SURVEILLANCE

 

With TBM Covid-19, TELBIOMED has developed an efficient monitoring system for the current Covid-19 pandemic. This was possible based on the long-time proven TBM Solutions and the modular structure of the system, which can be adapted relatively easily for certain disease patterns.

TBM Covid-19 supports health authorities in the care and management of people in quarantine. People in quarantine regularly record their body temperature and answer relevant questions regarding their symptoms. These data are available in a structured form for authorized caregivers.

Health professionals can quickly filter for persons with conspicuous symptoms. This supports an efficient and targeted use of resources in case of increasing numbers of quarantine cases.

Different user roles (coordinator, nurse, physician… ) and care groups (central, regional) support flexible management and ensure an efficent overview of the persons to be cared for.

AIT telehealth application wins the "Living Standards Awards 2018

 

On 17 January 2018, the internationally announced Living Standards Award was presented for the fourth time in Vienna. The prize honours exemplary participation in the standardisation and use of standards and is awarded to five favourites by an international jury of experts. Anton Dunzendorfer’s team from the Digital Health Information Systems Competence Unit of the AIT Austrian Institute of Technology was honoured for its consistent implementation of standards in telehealth applications. This enables us to seamlessly and securely integrate measuring devices for blood sugar, blood pressure, body weight, oxygen saturation, etc. used by people with chronic illnesses. Peter Kastner (Thematic Coordinator at AIT) commented: “By taking standards into account, it is possible to integrate various measuring sensors from different manufacturers in a standardised way via a device management system and to transfer the measured values simply and securely into a health app on the smartphone directly after recording. The data is then synchronized with a central server to which the attending physician has access”.

The AIT is regarded as a driving force for innovation in the Austrian healthcare sector when it comes to implementing new telehealth applications for the care of people with chronic illnesses such as diabetes mellitus, cardiac insufficiency or high blood pressure. In particular, the active support of often elderly people in the long-term therapy process is in focus. The IT services developed for this purpose (“DiabMemory”, “HerzMobil”, “DiabCare”, etc.), together with specific therapy management programs, enable a simple and new type of dialogue between doctors, health and care professionals and patients. “In the Health Dialogue of the Insurance Institution for Railways and Mining (VAEB), in which data from a wide range of terminal devices is recorded, we are thus offering users support for self-control and behavioural change,” says Kurt Völkl, General Director of VAEB, a long-standing partner of AIT in the cooperation within the framework of telehealth services.

The AIT takes standards into account from the outset of innovative developments to ensure sustainable use of the technologies.  Here, AIT is relying on standardisation initiatives such as the Personal Connected Health Alliance, the NFC Forum and the Bluetooth Special Interest Group. This ensures the seamless integration of measuring sensors from different manufacturers and interoperability across system boundaries and prepares for ELGA compatibility. The digital interaction between people and technology should contribute to a sustainable improvement in health care.
