HerzMobil Bluthochdruck - Digitale Gesundheitsversorgung mit messbarem Erfolg<\/h3>\nIm Rahmen eines innovativen Pilotprojekts wurde in Tirol ein digitales Disease Management Programm\u00a0f\u00fcr Patient:innen mit arterieller Hypertonie in Kooperation zwischen LIV Tirol<\/a>, Tirol Kliniken GmbH<\/a>, KUF, BVAEB - Versicherungsanstalt \u00f6ffentlich Bediensteter, Eisenbahnen und Bergbau<\/a> und dem AIT Austrian Institute of Technology<\/a> erfolgreich umgesetzt.<\/span><\/p>\n\u00dcber die TeleHealth-Plattform HerzMobil\u00a0wurden 151 Patient:innen \u00fcber 3 bis 6 Monate engmaschig von Pflegefachkr\u00e4ften und \u00c4rzt:innen betreut.<\/p>\nErgebnisse:\n\u2705\u00a0Blutdruck im Median signifikant gesenkt: \u201312 mmHg systolisch, \u20137 mmHg diastolisch\n\u2705\u00a064\u00a0% der Teilnehmer (n = 96) systolisch und 66 % der Teilnehmer (n\u00a0=\u00a0100) waren diastolisch kontrolliert\n\u2705\u00a0Signifikante Verbesserungen bei BMI und K\u00f6rpergewicht\n\u2705\u00a0Starke F\u00f6rderung der Gesundheitskompetenz durch Patientenedukation<\/p>\nDas Projekt belegt eindrucksvoll, wie Telemedizin, Telepflege, individuelle Patientenedukation und strukturierte, interdisziplin\u00e4re Betreuung\u00a0zu einer besseren Versorgung beitragen k\u00f6nnen.\nDie sehr gute Zusammenarbeit mit den Versicherungstr\u00e4gern BVAEB<\/a> und KUF<\/a> wird fortgesetzt und um die Zusammenarbeit mit der Sozialversicherung der Selbst\u00e4ndigen<\/a> erweitert.<\/p>\n\nHier geht es zur vollst\u00e4ndigen Publikation:\nhttps:\/\/www.kup.at\/kup\/pdf\/15711.pdf<\/a>\n<\/span><\/p>\n<\/p>\n<\/div><\/div>

Integrierte Versorgung f\u00fcr Menschen mit Epilepsie - TeleEpi-Konsortialmeeting in Zwolle<\/h3>\nAm 27. und 28. Mai traf sich das TeleEpi-Konsortium pers\u00f6nlich bei SEIN (Stichting Epilepsie Instellingen Nederland) in Zwolle (NL).<\/span><\/p>\nTeleEpi ist ein europ\u00e4isches Forschungsprojekt mit dem Ziel, einen neuen integrierten Versorgungspfad f\u00fcr Menschen mit Epilepsie zu entwickeln. \ud83e\udde0<\/span><\/p>\nDas Projekt fokussiert sich auf die Umsetzung einer telemedizinischen L\u00f6sung, die eine kontinuierliche Patienten\u00fcberwachung zu Hause erm\u00f6glicht und gleichzeitig wichtige ambulante und station\u00e4re Dienste integriert.\u00a0<\/span><\/p>\nW\u00e4hrend des zweit\u00e4gigen Treffens pr\u00e4sentierten die Partner von SEIN, AIT, Byteflies, CHUV, Karl Landsteiner Institut und telbiomed den aktuellen Stand ihrer jeweiligen Entwicklungen: Wearables, Telehealth-Plattform, Backend-Infrastruktur und ein digitales Anfallstagebuch.\u00a0<\/span><\/p>\nEin Rundgang in der SEIN-Klinik rundete das Programm ab. \ud83c\udfe5<\/span><\/p>\nDas Treffen bot eine wertvolle Gelegenheit, die Zusammenarbeit zu verst\u00e4rken und sich auf die n\u00e4chsten Phasen des Projekts zur Verbesserung der Epilepsieversorgung durch digitale Innovation vorzubereiten. \ud83d\udca1\u00a0<\/span><\/p>\n<\/p>\n<\/div><\/div>

Telemedizin bei pAVK: Neue Publikation zeigt Verbesserungen in Gehf\u00e4higkeit und Schmerzreduktion<\/h3>\nWir gratulieren unserem Kollegen\u00a0Fabian Wiesm\u00fcller<\/b>\u00a0von unserem Kooperationspartner AIT und den Co-Autoren zur erfolgreichen Ver\u00f6ffentlichung der Studie\u00a0\u201eSupport of Home-Based Structured Walking Training and Prediction of the 6-Minute Walk Test Distance in Patients With Peripheral Arterial Disease Based on Telehealth Data<\/a>\u201c<\/b>\u00a0im Fachjournal\u00a0JMIR Cardio<\/i>.<\/p>\nIn der prospektiven Kohortenstudie wurde untersucht, wie sich ein strukturiertes, telemedizinisch unterst\u00fctztes Gehtraining auf Patient:innen mit peripherer arterieller Verschlusskrankheit (pAVK) auswirkt.<\/div>\n<\/div>\nErgebnisse im \u00dcberblick:<\/b><\/div>\nHohe Adh\u00e4renz und Akzeptanz:<\/b>\u00a0Mit einer durchschnittlichen Durchf\u00fchrung von 85\u202f% der empfohlenen 6-Minuten-Gehtests zeigt sich eine hohe Adh\u00e4renz und damit eine starke Akzeptanz des telemedizinischen Trainingsprogramms.<\/div>\n\u00a0<\/p>\nVerbesserung der k\u00f6rperlichen Leistungsf\u00e4higkeit:<\/b>\u00a0Die zur\u00fcckgelegte Strecke beim 6-Minuten-Gehtest nahm im Verlauf der Studie signifikant zu, ebenso wie die Mobilit\u00e4t, die sich im Timed-Up-and-Go-Test deutlich verbesserte.<\/div>\n\u00a0<\/p>\nSchmerz und Wohlbefinden:<\/b>\u00a0Die berichtete Schmerzintensit\u00e4t nahm signifikant ab. Das subjektive Wohlbefinden zeigte eine positive Korrelation mit Gehstrecke und Schrittzahl.<\/div>\n\u00a0<\/p>\nPr\u00e4diktives Modell:<\/b>\u00a0Ein Regressionsmodell konnte die zur\u00fcckgelegte Strecke im 6-Minuten-Gehtest am Ende der Studie bereits zu Beginn mit einer Genauigkeit von 30 Metern vorhersagen.<\/div>\n\u00a0<\/p>\nDie Studie zeigt, wie telemedizinische Trainingsprogramme bei pAVK-Patient:innen die k\u00f6rperliche Leistungsf\u00e4higkeit verbessern und damit neue Ans\u00e4tze f\u00fcr die Versorgung dieser Erkrankung er\u00f6ffnen.<\/div>\n<\/div>\n<\/p>\n<\/div><\/div>

HerzMobil als nationales telemedizinisches Versorgungsprogramm f\u00fcr Patient:innen mit Herzinsuffizienz in \u00d6sterreich.<\/h3>\nIn \u00d6sterreich werden j\u00e4hrlich mehr als 14.000 Todesf\u00e4lle aufgrund einer Herzinsuffizienz verzeichnet und 24.000 Hospitalisierungen. Etwa 3 % der Gesamtbev\u00f6lkerung in \u00d6sterreich leiden an einer Herzinsuffizienz, wobei die Inzidenz mit dem Alter deutlich zunimmt. HerzMobil ist ein telemedizinisches und telepflegerisches Disease-Management Programm f\u00fcr Patient:innen mit Herzschw\u00e4che. Die Software-L\u00f6sung wurde von der AIT Austrian Institute of Technology GmbH<\/a> entwickelt und wird von telbiomed in der Regelversorgung serviciert. Der Nutzen von HerzMobil<\/a> konnte bereits durch zahlreiche Evaluierungen nachgewiesen werden. Nach Herzkreislauferkrankungen und Krebserkrankungen ist der Schlaganfall die dritth\u00e4ufigste Todesursache in \u00d6sterreich.<\/p>\nStrokeNet, ein Telehealth-Service f\u00fcr die ambulante Rehabilitation nach einem Schlaganfall, unterst\u00fctzt die mobile Rehabilitation und ist eine L\u00f6sung f\u00fcr die Koordination der integrierten, h\u00e4uslichen Versorgung von Patient:innen nach einem akuten Schlaganfallereignis. Der TBM CareManager unterst\u00fctzt eine enge Abstimmung zwischen allen beteiligten Partner:innen (\u00c4rzt:innen, Physio-und Ergo-Therapeut:innen sowie Logop\u00e4d:innen) unter Einbeziehung von Kliniken, Reha-Einrichtungen, SV-Tr\u00e4gern und Koordinationsstellen.<\/p>\nZiel des Versorgungsprogramms ist eine Verbesserung und Nachhaltigkeit des Behandlungserfolges f\u00fcr Schlaganfallbetroffene nach der Akutnachbehandlung im Krankenhaus, durch eine optimale Abstimmung der weiterf\u00fchrenden Behandlung im ambulanten Setting.<\/p>\n<\/div><\/div>
HerzMobil Tirol bringt vielf\u00e4ltige Vorteile f\u00fcr PatientInnen und Gesundheitssystem<\/div>
Das Versorgungsprogramm HerzMobil Tirol wird vom Landesinstitut f\u00fcr Integrierte Versorgung (LIV) Tirol koordiniert und umgesetzt. J\u00e4hrlich werden rund 350 PatientInnen nach ihrer Krankenhausentlassung durch HerzMobil Tirol versorgt. Finanziert wird\u00a0<\/span>HerzMobil Tirol durch Mittel des Landes Tirol, der Sozialversicherungstr\u00e4ger und Mittel aus dem Tiroler Gesundheitsfonds.<\/span><\/p>\nF\u00fcr HerzMobil Tirol sind aktuell 65 Netzwerk\u00e4rztInnen und 17 Pflegepersonen im Einsatz. Bei der Betreuung \u00fcber HerzMobil Tirol werden nicht nur Gesundheitswerte engmaschig \u00fcberwacht, sondern die PatientInnen zudem bei einer Umstellung hin zu einem ges\u00fcnderen Lebensstil unterst\u00fctzt.<\/p>\nDas Erfolgskonzept HerzMobil Tirol wird stetig weiterentwickelt. Projekt ADAPT: Durch Telemedizin Versorgungsqualit\u00e4t weiter steigern<\/div>
Das Programm HerzMobil wird bereits seit 2017 in der Steiermark umgesetzt. Nun wird an einem Softwaremodul gearbeitet, das die Versorgung f\u00fcr Herzschw\u00e4che-Patient*innen weiter verbessern soll. Das Projekt ADAPT wird vom Gesundheitsfonds Steiermark gef\u00f6rdert im Zuge des Digital!Healthcare-F\u00f6rdercalls, von telbiomed Medizintechnik und IT Service umgesetzt und erm\u00f6glicht eine noch gezieltere Medikamentendosierung. ADAPT steht f\u00fcr Automatisierte Dosisberechnung und Anzeige f\u00fcr Herzinsuffizienz Patient*innen in einem Telemedizinischen Versorgungsprogramm.\n\nADAPT soll in HerzMobil integriert werden und so dem Betreuungsteam die M\u00f6glichkeit geben, die aktuelle Medikamentendosierung mit den Empfehlungen der Leitlinie auf einen Blick zu vergleichen. Wusstest du, dass \u2026.<\/p>\n\netwa\u00a060 Millionen Menschen<\/a>\u00a0weltweit an einer Herzschw\u00e4che leiden?<\/li>\nalleine in \u00d6sterreich etwa\u00a0300.000 Menschen<\/a>\u00a0an einer Herzinsuffizienz leiden?<\/li>\nbis zu\u00a050 Prozent<\/a>\u00a0der Patienten nach einer Herzinsuffizienz-Hospitalisierung innerhalb von 5 Jahren versterben?<\/li>\nes pro Jahr ca.\u00a024.000 Hospitalisierungen<\/a>\u00a0aufgrund der Diagnose gibt?<\/li>\n<\/ul>\nDiese Hospitalisierungen verschlechtern nicht nur die Prognose der Patient:innen, die an einer Herzinsuffizienz leiden, sondern bedeuten auch erhebliche Kosten f\u00fcr das Gesundheitssystem.<\/p>\nUm diesem Problem zu begegnen und die Lebensqualit\u00e4t von Menschen mit einer Herzschw\u00e4che zu verbessern, wurde das telemedizinische Versorgungsprogramm\u00a0HerzMobil<\/a>\u00a0entwickelt.<\/p>\nHerzMobil f\u00fchrt zu einem besseren Umgang mit der Erkrankung, bei zugleich weniger Krankenhausaufenthalten. Mitte Februar 2024 ver\u00f6ffentlichte das Pharmaunternehmen\u00a0Janssen-Cilag GmbH<\/a>\u00a0ein Open House Positionspapier mit dem Titel \u201eValue Based Healthcare: Gesundheitsversorgung am Outcome f\u00fcr die Patient:innen ausrichten<\/strong>\u201e, welches gemeinsam mit interdisziplin\u00e4ren Expert:innen aus dem Gesundheitswesen erarbeitet wurde. Im Positionspapier formulieren die Autor:innen Forderungen an die Politik, um das deutsche Gesundheitssystem im Sinne von Value Based Healthcare (VBHC) weiterzuentwickeln.<\/p>\nValue Based Healthcare<\/strong>\u00a0orientiert sich am Nutzen und der erzielten Gesundheit des Patienten. Telbiomed beim Grazer Marathon<\/div>
Gemeinsam haben wir die Distanz von 42,5 km als 4er-Team bew\u00e4ltigt und haben in der Disziplin \u201eHyundai Staffelmarathon\u201c in der Kategorie Mixed den 32. Platz geschafft mit einer Gesamtzeit von 3 Stunden und 40 Minuten. Wir sind mehr als zufrieden mit unserer Leistung und freuen uns schon auf das n\u00e4chste Jahr.<\/p>\n<\/p>\n<\/div><\/div>

Obwohl der Fokus bisher auf Forschungsprojekten und klinischen Studien lag, wurden w\u00e4hrend der Pandemie im South Eastern Sydney Local Health District (SESLHD) mit der KIT Telehealth Plattform \u00fcber 8.000 Cov19-F\u00e4lle monitiert. Ergebnisse der bisherigen Partnerschaft wurden am 27. Patient*innen werden dabei von Pflegekr\u00e4ften geschult und gemeinsam mit \u00c4rzt*innen im Versorgungsnetzwerk betreut. Medizinisches Fachpersonal kann anhand der Daten zeitnah die Therapie (etwa hinsichtlich Medikation und Dosierung) optimieren.<\/p>\nEine aktuelle Evaluierung von HerzMobil in Tirol zeigte bei Herzschw\u00e4che Patient*innen eine 61% niedrigere, j\u00e4hrliche Sterbeh\u00e4ufigkeit und eine 34% niedrigere Wiederaufnahmerate im Vergleich zur Standardversorgung.<\/p>\nDer telemedizinische Behandlungspfad HerzMobil wird bereits in 3 Bundesl\u00e4ndern angewendet (siehe\u00a0www.herzmobil.at<\/a>). M\u00f6glich war das basierend auf den langj\u00e4hrig bew\u00e4hrten TBM Solutions und des modularen Aufbaus des Systems, der relativ einfach f\u00fcr bestimmte Krankheitsbilder adaptiert werden kann.<\/p>\nTBM Covid-19 unterst\u00fctzt Kliniken und die Gesundheitsbeh\u00f6rden bei der Betreuung und dem Management von Personen in Quarant\u00e4ne. Damit wir eine nahtlose und sichere Einbindung von Messger\u00e4ten f\u00fcr Blutzucker, Blutdruck, K\u00f6rpergewicht, Sauerstoffs\u00e4ttigung, u.a., die von Personen mit chronischen Erkrankungen verwendet werden, erm\u00f6glicht. Peter Kastner (Thematischer Koordinator bei AIT) dazu: \u201eDurch Ber\u00fccksichtigung von Standards ist es m\u00f6glich, verschiedene Messsensoren von verschiedenen Herstellern \u00fcber ein Ger\u00e4temanagement standardisiert einzubinden und die Messwerte direkt nach der Erfassung einfach und sicher in eine Gesundheits-App am Smartphone zu \u00fcbernehmen. Die Daten werden dann mit einem zentralen Server synchronisiert, auf den der betreuende Arzt Zugriff hat.\u201c<\/p>\nDas AIT gilt im \u00f6sterreichischen Gesundheitswesen als Innovationsmotor wenn es um die Umsetzung neuer Telegesundheitsanwendungen f\u00fcr die Betreuung von Personen mit chronischen Erkrankungen wie Diabetes mellitus, Herzschw\u00e4che oder Bluthochdruck geht. Speziell die aktive Unterst\u00fctzung der oftmals \u00e4lteren Personen beim Langzeit-Therapieprozess steht dabei im Fokus. On May 27th and 28th, the TeleEpi consortium met for an in-person meeting at SEIN (Stichting Epilepsie Instellingen Nederland) in Zwolle (NL).<\/p>\nTeleEpi is a European research project aiming to develop and evaluate a new integrated care pathway for people with epilepsy.\u00a0 The project focuses on implementing a telemedical solution that enables continuous, unobtrusive patient monitoring at home while integrating essential outpatient and inpatient services.<\/p>\nDuring the two-day meeting, partners from SEIN<\/a>, AIT<\/a>, Byteflies<\/a>, CHUV<\/a>, Karl Landsteiner Institute<\/a> and telbiomed presented updates on their respective developments.The agenda covered demonstration of wearables, telehealth platform, backend infrastructure and a digital seizure diary. Telemedicine for PAD: New publication shows improvements in walking ability and pain reduction<\/h3>\nWe congratulate our colleague Fabian Wiesm\u00fcller from our cooperation partner AIT and the co-authors on the successful publication of the study \u201cSupport of Home-Based Structured Walking Training and Prediction of the 6-Minute Walk Test Distance in Patients With Peripheral Arterial Disease Based on Telehealth Data<\/a>\u201d in the journal JMIR Cardio.<\/p>\nThe prospective cohort study investigated the effects of structured, telemedicine-supported walking training on patients with peripheral arterial disease (PAD).<\/p>\nResults at a glance:\n<\/strong>\nHigh adherence and acceptance:<\/strong> With an average performance of 85% of the recommended 6-minute walking tests, there is a high level of adherence and thus a high level of acceptance of the telemedical training program.<\/p>\nImprovement in physical performance:<\/strong> The distance covered in the 6-minute walk test increased significantly over the course of the study, as did mobility, which improved significantly in the timed-up-and-go test.<\/p>\nPain and well-being:<\/strong> The reported pain intensity decreased significantly. Subjective well-being showed a positive correlation with walking distance and number of steps.<\/p>\nPredictive model:<\/strong> A regression model was able to predict the distance covered in the 6-minute walk test at the end of the study with an accuracy of 30 meters at the beginning.<\/p>\nThe study shows how telemedical training programs can improve the physical performance of PAD patients.<\/p>\n<\/p>\n<\/div><\/div>

Around 3% of the total population in Austria suffer from heart failure, with the incidence increasing significantly with age. HerzMobil is a telemedical and telecare disease management program for patients with heart failure. The software solution was developed by AIT Austrian Institute of Technology GmbH<\/a> and is serviced by telbiomed in standard care. The benefits of HerzMobil<\/a> have already been proven by numerous evaluations. A stroke occurs every 20 minutes. Around 25,000 Austrians<\/a> suffer a stroke every year. After cardiovascular disease and cancer, stroke is the third most common cause of death in Austria.<\/p>\nStrokeNet, a telehealth service for outpatient rehabilitation after a stroke, supports mobile rehabilitation and is a solution for coordinating integrated, home-based care for patients after an acute stroke event. The TBM CareManager supports close coordination between all partners involved (doctors, physiotherapists, occupational therapists and speech therapists) with the involvement of clinics, rehabilitation facilities, social insurance providers and coordination centers.<\/p>\nThe aim of the care program is to improve and sustain the success of treatment for stroke patients after acute follow-up treatment in hospital by optimally coordinating further treatment in an outpatient setting.<\/p>\n<\/div><\/div>
HerzMobil Tirol brings many benefits for patients and the healthcare system<\/div>
The HerzMobil Tirol care program is coordinated and implemented by the Tyrolean State Institute for Integrated Care (LIV). Every year, around 350 patients are cared for by HerzMobil Tirol after being discharged from hospital. HerzMobil Tirol is financed by the state of Tyrol, the social insurance institutions and funds from the Tyrolean Health Fund.<\/p>\nHerzMobil Tirol currently employs 65 network doctors and 17 nursing staff. The care provided by HerzMobil Tirol not only involves close monitoring of health values, but also supports patients in changing to a healthier lifestyle.<\/p>\nThe successful HerzMobil Tirol concept is constantly being developed further. A new traffic light system for the participating network doctors is intended to further improve the quality of therapy and, in particular, the adjustment of medication. \u201cThe traffic light system shows the treating doctors the quality of the current therapy on a daily basis. This allows for an even more targeted response and optimization. ADAPT project: further improving the quality of care through telemedicine<\/div>
The HerzMobil program has been implemented in Styria since 2017. Work is now underway on a software module to further improve care for heart failure patients. The ADAPT project is funded by the Styrian Health Fund as part of the Digital!Healthcare funding call, implemented by telbiomed Medizintechnik and IT Service and enables even more targeted medication dosing. ADAPT stands for Automated Dose Calculation and Display for Heart Failure Patients in a Telemedical Care Program.\n\nADAPT is to be integrated into HerzMobil, enabling the care team to compare the current medication dosage with the guideline recommendations at a glance. The guideline-compliant dosage should further improve patient care.<\/div><\/div>

Did you know that ....<\/p>\n\naround 60 million people<\/a> worldwide suffer from heart failure?<\/li>\naround 300,000 people<\/a> in Austria alone suffer from heart failure?<\/li>\nup to 50 percent<\/a> of patients die within 5 years of being hospitalized for heart failure?<\/li>\nthere are around 24,000 hospitalizations<\/a> per year due to the diagnosis?<\/li>\n<\/ul>\nThese hospitalizations not only worsen the prognosis of patients suffering from heart failure, but also mean considerable costs for the healthcare system.<\/p>\nThe telemedical care program HerzMobil was developed to tackle this problem and improve the quality of life of people with heart failure.<\/p>\nHerzMobil leads to better management of the disease and fewer hospital stays. Further advantages of the telemonitoring program:<\/p>\n\nRelief for the inpatient sector<\/li>\nReduction in healthcare expenditure<\/li>\nReduction in avoidable hospital admissions<\/li>\nReduction in mortality (25.7% to 10.0%<\/a>)<\/li>\n<\/ul>\nHerzMobil is currently financed by the respective regional health funds and social insurance in Tyrol, Styria, Carinthia and Lower Austria. In mid-February 2024, the pharmaceutical company Janssen-Cilag GmbH<\/a> published an open house position paper entitled \u201cValue Based Healthcare: Aligning healthcare with patient outcomes\u201d, which was developed together with interdisciplinary experts from the healthcare sector. In the position paper, the authors formulate demands for policymakers to further develop the German healthcare system in line with value-based healthcare (VBHC).<\/p>\nValue-based healthcare is based on the benefits and the health achieved by the patient. The quality of the outcome is compared with the costs in order to achieve the best possible patient benefit. The guiding principle is \u201coutcome over cost\u201d.<\/p>\nWe are delighted that the authors have cited the HerzMobil Tirol<\/a> program as a best practice example for the national and international arena! LebensGro\u00df is a new cooperation partner of telbiomed<\/div>
The \u201cDiLeLi\u201d project by LebensGro\u00df was chosen as the winner of the \u201cDigital!Healthcare\u201d funding call<\/a> by the Styrian Health Fund.<\/p>\nIn 2021, LebensGro\u00df GmbH opened Austria's first day center for people with eating disorders (LeLi) in Graz. Results of the partnership to date were presented on July 27 in the special session \u201cDigital Health Innovation - from Telehealth to Data-Driven Health and Care\u201d at the IEEE Engineering in Medicine and Biology Society Conference (IEEE EMBC 2023)<\/a> by G\u00fcnter Schreier (Senior Scientist, AIT), Sze-Yuan Ooi (Cardiologist, PoWH), Peter Kastner (CEO, telbiomed) and Ahmadreza Argha (Scientist, UNSW) in Sydney.<\/p>\nIn the discussion during the subsequent visit to the PoWH Virtual Care Center, the need for digital health innovations to overcome the challenges facing healthcare systems was discussed. Improving care for the chronically ill and supporting follow-up care after inpatient treatment are also high priorities in Australia.<\/p>\n<\/p>\n\nAIT and TBM as Exhibitors at the 13th Austrian Health Industry Congress<\/strong><\/h3>\nOn June 22 and 23, 2023, the 13th Austrian Health Industry Congress (\u00d6GWK)<\/a> took place in Vienna. Together with the AIT Austrian Institute of Technology<\/a>, we participated as exhibitors. We had the pleasure of welcoming Prof. Dr. Elke Guenther (Head of Center for Health & Bioresources) as well as one of AIT\u2019s new managing directors, Mag. Success Story of IHE Austria - Standardized Telemonitoring Episode Report for ELGA<\/h3>\nFind out more about the standardized telemonitoring episode report for ELGA in the new success story<\/a> from IHE Austria.<\/p>\nWith the telemonitoring episode report, telemedical care programs for chronic diseases are connected to the electronic health record (ELGA).\nThe episode report was developed by telbiomed Medizintechnik und IT Service GmbH and AIT Austrian Institute of Technology GmbH<\/a> with the support of HL7 Austria<\/a>, ELGA GmbH<\/a> and other partners. Using the collected data, medical professionals can promptly optimize treatment, particularly regarding medication and dosage.<\/p>\nA recent evaluation of HerzMobil in Tirol showed a 61% lower annual mortality rate<\/strong> and a 34% lower hospital readmission rate<\/strong> for heart failure patients compared to standard care.<\/p>\nThe telemedical care pathway HerzMobil<\/em> is already in use in three Austrian provinces (see www.herzmobil.at<\/a>). This was made possible by the long-established TBM Solutions<\/em> and the system\u2019s modular design, which allows for relatively simple adaptation to specific medical conditions.<\/p>\nTBM Covid-19<\/em> supports clinics and public health authorities in the care and management of individuals in quarantine. People in quarantine regularly record their body temperature and respond to relevant health questions. The award honors outstanding contributions to the development and application of standards, and is granted by an international expert jury to five selected winners<\/a>. The team led by Anton Dunzendorfer from the Competence Unit Digital Health Information Systems<\/em> at the AIT Austrian Institute of Technology<\/em> received the award for their consistent implementation of standards in telehealth applications.<\/p>\nTheir work enables seamless and secure integration of medical measurement devices\u2014such as for blood glucose, blood pressure, body weight, and oxygen saturation\u2014used by individuals with chronic conditions. Peter Kastner, thematic coordinator at AIT, explains:\u201cBy adhering to standards, we can integrate various sensors from different manufacturers using a standardized device management system, allowing measured values to be securely and easily transferred to a health app on a smartphone immediately after capture. The data is then synchronized with a central server accessible by the attending physician.\u201d<\/p>\nAIT is regarded as a key driver of innovation in Austria\u2019s healthcare system, particularly in developing new telehealth applications for managing chronic conditions such as diabetes, heart failure, or hypertension. Special attention is given to supporting older patients in their long-term therapy processes. The IT services developed for this purpose\u2014such as DiabMemory<\/em>, HerzMobil<\/em>, and DiabCare<\/em>\u2014along with specific therapy management programs, enable a new and simplified form of communication between physicians, healthcare professionals, and patients.\u201cIn the Health Dialogue<\/em> program of the Insurance Institution for Railway and Mining Employees (VAEB)<\/em>, where data from a variety of end devices is collected, we support users in self-monitoring and behavior change,\u201d says Kurt V\u00f6lkl, CEO of VAEB, a long-standing AIT partner in telehealth services.<\/p>\nFrom the outset, AIT integrates international standards into its innovative developments to ensure long-term usability and scalability of the technologies.
